Finally! Thank you! The climax of the movie is that the private eye learns that the long vanished crooner he is looking for wanted to find a way out from his (the crooner's) deal with the Devil. He realizes that the crooner's plan was to take some random stranger, kill him and eat his heart and absorb his identity. And the private eye learns that the man the crooner killed was himself. He isn't the private eye at all, but the crooner who thinks he is the private eye. He is his own murderer.
Also the director was Englishman Alan Parker. The movie stars Robert De Niro, who had won the best Actor Oscar six years earlier, and Mickey Rourke and Charlotte Rampling, who would both be nominated for lead actors this century, but both lose. The disturbing sex scene involves the New Orleans downpour outside somehow turning into blood inside. Later we learn that this incident was (unknowing) incest.
Had this not been enough, I would have mentioned that a cauldron of gumbo serves as a murder weapon. I might have mentioned that of the six lead actors there are five Americans and an English woman playing an American (Rampling), while of the six lead characters, there are five Americans and one pretending to be an American (De Niro).

OH yeah, 'Angel Heart'. Never saw it, but I do now recall the controversy that surrounded a young Lisa Bonet's part in this film, and something about how Bill Cosby put his two cents in about it at the time.

THE OTHER is not the movie.
The "twins" in the movie in question are adults, not children. Also, the death in the movie in question is not perceived as an accidental as is the case in THE OTHER.

Correct, the movie is Brian De Palma's SISTERS with Margot Kidder as the "twins"
